Relationship between Ketones, Ghrelin, and, Appetite on Isocaloric Diets with Varying Carbohydrate Quality and Amount: Results from a Randomized Controlled Trial in People with Obesity (CARBFUNC).
RCT of 193 adults with obesity comparing isocaloric diets with varying carbohydrate quality and quantity. The study evaluated changes in fasting plasma concentrations of ghrelin, β-hydroxybutyrate, and subjective appetite. No significant differences in fasting ghrelin or hunger feelings were found between diets, though β-hydroxybutyrate increased more with the LCHF diet.

A randomized, placebo-controlled crossover trial involving 20 women with PCOS examined the effects of β-hydroxybutyrate (BHB) supplementation. BHB supplementation acutely lowered circulating androgen and glucose levels, suggesting potential for ketone-based therapies in PCOS treatment.

A randomized, double-blind, clinical pilot trial investigated the effects of ketone salt supplementation on PTSD symptoms in 21 participants. The ketone group showed a significant decrease in PTSD Checklist for DSM-5 (PCL-5) scores from pretest to posttest, although changes were not statistically significant between groups. Larger trials are needed to confirm these findings.
The Combined Effects of Exercise, Diet, and a Multi-Ingredient Dietary Supplement on Body Composition and Adipokine Changes in Overweight Adults.
This randomized, placebo-controlled, double-blind study evaluated the effects of an 8-week diet and exercise program combined with a dietary supplement containing raspberry ketone, capsaicin, caffeine, garlic, and Citrus aurantium on body composition and adipokines in overweight adults. Significant decreases in weight, fat mass, and percentage body fat, and increases in lean body mass were observed. The supplement group showed greater improvements in adipokines, with decreased leptin and increased adiponectin levels.
The Acute Effects of Nonstimulant Over-the-Counter Dietary Herbal Supplements on Resting Metabolic Rate.
Random-repeated measures crossover study with 26 participants testing the acute effects of raspberry ketones and a metabolic activator blend on resting metabolic rate and substrate utilization. No significant differences were found in RMR, respiratory quotient, or substrate utilization between the supplements and placebo.
