The Pharmacokinetic Interaction Between Metformin and the Natural Product Goldenseal Is Metformin Dose-Dependent: A Three-Arm Crossover Study in Adults With Type 2 Diabetes.
This three-arm, open-label, crossover clinical study assessed the pharmacokinetic interaction between goldenseal and metformin in adults with type 2 diabetes. The study found that goldenseal decreased metformin systemic exposure at lower doses, but the overall interaction was not clinically meaningful. The study highlights the need to evaluate clinical biomarkers for drug interaction risks involving natural products.

Effect of goldenseal (Hydrastis canadensis) and kava kava (Piper methysticum) supplementation on digoxin pharmacokinetics in humans.
RCT with 20 healthy volunteers to assess the effect of goldenseal and kava kava supplementation on digoxin pharmacokinetics. Goldenseal showed a 14% increase in C(max), but no other significant effects on digoxin pharmacokinetics were observed. The study suggests these supplements are not potent modulators of P-gp in vivo.

RCT with 12 healthy volunteers assessing the effects of goldenseal, kava kava, black cohosh, and valerian on CYP enzyme activity. Goldenseal significantly inhibited CYP2D6 and CYP3A4/5 activity, kava kava inhibited CYP2E1, and black cohosh weakly inhibited CYP2D6. Valerian showed no significant changes in enzyme activity.
Assessing Transporter‐Mediated Natural Product‐Drug Interactions Via In vitro‐In Vivo Extrapolation: Clinical Evaluation With a Probe Cocktail
The study evaluated the effects of goldenseal, standardized to berberine, on transporter activity using in vitro systems and a clinical probe cocktail in 16 healthy volunteers. Goldenseal increased the AUC of midazolam but had no effects on rosuvastatin and furosemide pharmacokinetics, while decreasing metformin AUC, suggesting altered intestinal permeability and transport.
